At MOS quality is paramount. We take pride in the QA process that we follow during the Medical Records Review process. Our multi level quality check process allows our staff to minimize errors and provide excellent reports. Since we understand how complicated the documentation can be, coming from multiple providers which can be typed, handwritten with quality issues we use a document management system which allows us to sort easily and capture the required medical chart information quickly for review.

We realize the importance of quality and extend this outlook to our production process to assure accuracy in the data returned. Our MIS department tracks all data that comes in to our office with accurate tracking.

Our QA Process

Every review returned is put through a multilevel quality process. The quality of individuals who can be involved will depend on the project and scope of the project. We presently employ paramedical and medical staff (Therapists, Nurses and Physicians) who provides the quality that is expected by our clients. This happens during the analysis of the documents and documenting as well as during the abstraction of data sets for chart review.

• The production floors will do the first level of checking of all documents they worked on.
• Second level of QA is done by reviewers who will go through all documents which would then be passed on or sent back for further checking and corrections.
• Third level of QA is done as an over sampling of all documents done at a minimum of 5% by a medical practitioner for accuracy.
• The documents are then sent to the MIS department for review of file name accuracy; naming of the file and TAT of the file is maintained.
